Maintenance Request Form
Use this form to submit requests for maintenance in University-owned residential properties. For maintenance requests that are not in residential areas, please contact Physical Plant.
This form is for residents of Stambaugh, Lima Complex, Clark, and Affinity who may register weekend guests.
