Sales Tax Exemption Policy
State of Ohio Sales Tax Rules

STATE FORMS: 

Each of the following forms are set up so that you can insert the necessary information and then print the form. 

Colorado FYI - Will accept Ohio form for occasional use

Florida - use as is

Indiana - add description of items purchased and current date

Iowa - not applicable for meals or lodging

Kentucky - fill in vendor name and address, description of property or servcie, date 

Maryland - application denied, must be an adjacent state 

Michigan - fill in vendor name & address and date; include copy of IRS letter (pgs. 2-3)

Minnesota - not applicable for meals or lodging

Missouri - use as is

New Jersey - add date, vendor name & description of purchase

New York - check blanket certificate, fill in vendor name and address

Ohio - fill in the name of the merchant or vendor, sign your name legibly, include your title or position at the university, and add the date.  Any university employee authorized to make a purchase for ONU may sign the form. 

Pennsylvania - fill in vendor name and address, sign and date 

Virginia - not applicable for meals or lodging

Wisconsin - Out of state shools do not qualify for exemption of sales tax
